Registration/Renewal Form

The normal annual individual membership fee for 2010 is US$50 and includes a print subscription to Volume 4 of the Journal of Mathematics and Music.

New members may register at any time. Membership fees received on or before May 31, 2010 will be applied to a 2010 membership. Membership fees received after that date will be applied to a 2011 membership and subscription to Volume 5 of JMM.

New members who would like to subscribe retroactively for the 2009 subscription year and receive Volumes 3 and 4 of the journal may do so by paying an extended membership fee of US$90.

In order to complete your membership you need to submit your registration by filling out the following form and clicking the "submit" button. You must then use the "Pay Now" button on the next page to pay your membership fee. This requires you to register first as a PayPal user. If you wish to pay by check drawn on a US bank, please contact the treasurer of the society at [click to reveal]@gmail.com.

In accordance with the Memorandum of Understanding between the Society of Mathematics and Computation in Music and Taylor and Francis each subscriber agrees with the following regulation:
This privileged print subscription rate for Journal of Mathematics and Music is offered to individual members on the strict conditions that such subscriptions are for the private use of each member; received at the private address of that individual member; and copies are not to be placed in a library nor in any way used to substitute for an existing or potential library subscription. Discounted subscriptions apply to print copies only – online access is not included in the offer. Failure to abide by the above terms and conditions will result in immediate termination of the subscription.    I understand and accept the terms and conditions as detailed above.
